A good gift, for any occasion, is something you know the other person will like. It may sound obvious, but a lot of people just go for the conventional gifts at this time - flowers and chocolates, for instance - which is great if you know your wife will like them, but otherwise it can look as if you haven't really thought about it.

Is she a romantic person? Would she enjoy a restaurant meal, an evening in with a DVD, or would she prefer something like sexy underwear (be careful though; men and women often have different ideas about that!) Alternatively, how about getting something more long lasting? Jewellery can be tricky, but a simple yet elegant watch is a safe bet, or a bottle of a special perfume that you know she likes.

There are some gift ideas here that may give you inspiration; it's also worth thinking about a special trip if that's at all possible - expensive perhaps, but it certainly shows you care! (Though personally I would wait till after Valentine's and go for a weekend trip later.)
